A member must have paid by a contributing employer on his or her behalf to
the Fund, at the rate agreed to in the relevant Collective Bargaining
Agreement, 450 hours within a 4 consecutive month period to be covered
the first day of the fourth month after working the required 450 hours. For
example, if the 450th hour worked is in January then coverage would begin
May.
To add dependents, a copy of marriage certificate, birth certificate & Social
Security card is necessary.
$300. -----In patient hospital deductible
$50. ------ Out-patient hospital co-pay
$30. ------ Doctor co-pay
$200. ----- Major Medical deductible
